Following this conversation, eligible applicants will be invited to complete the full scholarship application through our secure patient portal. At that time, applicants will provide more detailed personal, financial, and treatment-related information for scholarship consideration.

Please note that all scholarship applicants are required to submit a nonrefundable $50 scholarship application fee once they have been approved to move forward with the formal application process.

Scholarship recipients will be notified via email once a decision has been made. The notification will include the amount of scholarship funding awarded, if applicable.
